Advance Care Planning (ACP) is a process of understanding, reflecting on and discussing what matters most to you.  It also helps prepare those closest to you for a time when you might be unable to speak for yourself.  ACP helps you make sure your goals and wishes for medical care can be known, honored and incorporated into the care you receive. ACP helps you to create a written plan (health care directive).  This document allows you to outline your medical preferences and name a “health care agent”, the trusted person you would want to make medical decisions if ever you couldn’t speak for yourself.

Most people hope to stay healthy and feel well for as long as possible.  However accidents or sudden illnesses can happen, so it is important to have a say in your care in advance.  The best time to make healthcare decisions is when you are able to make your own choices.
